Your job is practical, moral and ritualistic all at once. Plot routes, raise miners for fuel, keep valves and pipes from bursting and pull as many survivors aboard as you can. All while January repeats itself and the Pontiff General watches over salvation.

The core loop is straightforward and tense. From the command deck you select which city to attempt a rescue from and chart the route the IRON SANCTUARY will take. You will rely on dispatch data from higher command, instrument readings and a mix of practical tools and occult paraphernalia. Use the compass to circle areas on the map, place markers to mark hazards and calculate distances to estimate fuel burn and travel time.
Decisions matter broadly and intimately. Longer routes may reach more survivors but increase the risk of stalling. Breakdowns on board consume time and resources. Each passing day raises the death toll in the regions you leave behind. There is no perfect path, only the one you choose under pressure.

A large part of the game happens inside devastated cities. Your crew must raise giant drill miners and release cart ways that channel fuel back to the IRON SANCTUARY. That fuel must last the journey. Terrain will fight you. Demolished landscapes, blistering fields and inhuman temperatures create hazards that can damage the train.
Onboard you will be responsible for maintenance and emergency repairs. Secure pressure valves, swap broken pipe parts and keep every mechanism running at speed. Failures force you to stop and fix things in the field which costs days and lives. The game emphasizes resource triage and small engineering choices as much as route planning.

The setting leans hard into a specific kind of dread. The 1920s have been warped by the Great War into a place where hell overlaps Earth and the month of January repeats itself. As conductor you endure routine and mounting mental strain. The game suggests that the nightmares you face may not be consequences of your choices alone.
That does not mean there is no comfort. Small rituals matter. Watering Hope before a journey is an actual mechanic and a rare human touch. The ambient details help sell the atmosphere. Sit with a gramophone, listen to the oil-stained carriage creak and keep twitching systems in line while propaganda about the Pontiff General filters through the speakers. The developers cite influences such as IRON NEST, PVKK, the art direction of Dishonored and the atmosphere of BioShock, which shows in the game's heavy mood and world design.
When the route is set, the tanks are topped and safety checks are complete you descend into the control pit and ignite the engine. Rescue as many people as you can, but do so with an eye for practical survival. Every choice on the map affects fuel, mechanical health and the grim tally of lives at stake.
IRON SANCTUARY is not a simple hero simulator. It is a steady, sometimes punitive test of planning under pressure with a strong sense of place.
